I don't have the GISP player's banzuke, but to get the entry form you 
click the link that says "Entry Form" (it's in the text in the middle 
of the page) at www.gunga.com.  (the direct URL is 
http://www.gunga.com/form29685/form.html )

It's a different entry form than the one before, and you make your 
selections via radio buttons (non-maegashira) and check boxes 
(maegashira). When you're done, click submit at the bottom.
